Eastwood Primary Care Centre is seeking an enthusiastic, highly-motivated and experienced Clinical Pharmacist to join our friendly practice team and contribute to improving healthcare to our local community, with which we have strong links. The post-holder would be working within a practice that has a CQC outstanding rating, and we pride ourselves on supporting our patients and our staff.
The post-holder will either hold the independent prescribing qualification enabling them to work autonomously in practicing and prescribing safely and effectively within the primary care setting, or be willing to undertake the independent prescribing qualification as experience within primary care increases.
The post-holder will undertake clinics for clinical medication reviews to proactively manage patients, including those with complex polypharmacy and long-term conditions. They will also be required to undertake medicines reconciliation on transfer of care and will contribute to and manage some aspects of the quality and outcomes framework.
The post-holder will provide support to general practice staff and our specialised prescriptions team with regards to medication queries.
The post-holder will be required to provide clinical leadership on medicines optimisation and quality improvement and will contribute to training and education of junior pharmacists within the team.
The post-holder will be required to contribute to the seasonal vaccination programmes, of which the practice has served the community extremely well with in-house clinics and for our housebound patients.
EPCC is a well-established training practice and the post-holder would be working alongside a team of practice-based and PCN pharmacists, PCN pharmacy technicians and student pharmacists, along with being an integral part of the wider general practice team.
Eastwood Primary Care Centre, located on the border of Nottinghamshire/Derbyshire, sits at the heart of the community and we contribute to the wider health of Nottinghamshire and work closely with Primary Integrated Care Services, Nottingham West PCN and The University of Nottingham.
Our practice offers a professional and caring service to approximately 18,500 patients, utilising the skills and enthusiasm of our practice and wider team.
PRIMARY DUTIES AND AREAS OF RESPONSIBILITY:

The post holder will:
Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
Minimum of 2 years experience as a pharmacist, demonstrated within a practice portfolio.
Has experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice.
Will hold an independent prescribing qualification.
Recognises priorities when problem-solving and identifies deviations from normal patterns and is able to deal with or refer when appropriate.
Able to follow legal, ethical, professional and organisational policies/procedures and codes of conduct.
Involves patients in decisions about prescribed medicines and supporting adherence as per NICE guidelines.
Demonstrate understanding of the pharmacy role in governance and is able to implement this appropriately within the workplace.
Demonstrate understanding of, and contributes to, the workplace vision.
Engage with Patient Participation Groups (PPGs) and involves PPGs in development of the role and practices.
Demonstrate ability to improve quality within limitations of service.
