An established industry player is seeking a Clinical Pharmacist to enhance patient care through effective medicines management. Banbury Cross PCN is recruiting a Clinical Pharmacist who will operate as part of our practice team. The Pharmacist will work within designated, established practice teams, using and sharing pharmacy expertise to support GPs and the entire practice in effective medicines management and optimal patient care.
PML is a successful not-for-profit, GP-led organisation providing NHS community and primary care clinical services across Oxfordshire and Northamptonshire. Since 2004, we have grown significantly, employing around 300 staff with a turnover of approximately £16m. We hold GMS contracts and represent about 50 GP practices covering roughly 650,000 patients.

Medication Review and Optimisation:
Respond to reception queries to amend prescriptions, re-authorise repeats, clarify doses and products, and provide alternatives when availability issues arise. Discuss specific patient and prescribing issues with clinicians, conduct face-to-face clinics, and communicate medication issues with patients via telephone. Review secondary care requests for new medications, monitor and inform colleagues about prescribing issues, and advise on cost-effective prescribing.
Prescribing Systems and Policies:
Collaborate with staff to develop and implement medicines management systems and prescribing policies. Review and monitor prescribing performance, support electronic prescribing, and suggest improvements.
Working relationships and communication requirements:
Communicate effectively with patients, carers, colleagues, and agencies, providing complex information with empathy and respecting confidentiality. Manage conflicts and maintain professional standards in accordance with legal, ethical, and organizational policies.
Registered health professional:
Ensure adherence to professional standards of practice and conduct as set by relevant bodies.
Information governance:
Maintain high standards of record-keeping, confidentiality, and data protection in line with policies and legislation.
Confidentiality:
Uphold confidentiality and access information only as necessary for duties, promoting equality and diversity.
