Job summary

St Giles Hospice is seeking a Clinical Nurse Specialist for the Rapid Response Service to prioritise and triage patients with palliative, end-of-life and acute needs. Responsibilities

• Prioritise and triage patients presenting with a range of conditions, including palliative, EOL and acute cases.
• Provide personalised care planning, specialist treatment plans, ReSPECT and advanced care planning.
• Support specialist palliative care for patients and families in the home or usual place of care.
• Liaise with external services, community teams and virtual ward teams to ensure timely assessment and safe community-based management.
