Job description

The Community Chronic Pain Service is a multidisciplinary team of expert chronic pain clinicians. We support people with persistent pain to develop self-management strategies, enabling them to live well. This post is suitable for a Clinical Nurse Specialist or an Allied Health Professional with an interest in pain management.

Our aim is to help individuals develop the skills needed to improve their quality of life and reduce their dependency on healthcare services and medication.

This service is delivered in a variety of settings, including face-to-face clinics, group settings, and virtually.

In addition to community clinics, you will deliver care within the secure (prison) environment. We provide our award-winning Pain Management Service in all Kent prisons; therefore, travel will be an essential part of this post.

Key Responsibilities:
  1. Assessing, planning, implementing, and evaluating holistic care in relation to care planning and pathways.
  2. Developing integrated care programmes in partnership with service users, commissioners, and key stakeholders.
  3. Ensuring a supervision framework is in place for staff under your care and supporting staff in developing relevant competencies across staff groups.
  4. Actively working as a member of the integrated team to ensure high-quality care is provided to service users.
  5. Maintaining your own specialist area of competence and leading on the competency frameworks for others.
  6. Synthesising information to present proposals effectively to decision-makers (including Directors and Senior Executives) in ways that facilitate understanding and influence pathways.
  7. Making recommendations to decision-makers to drive forward innovative solutions, service improvements, and financial efficiencies related to prescribed care packages or projects.

Rated ‘outstanding’ by the CQC, we are passionate about delivering high standards of patient care and excellent services to improve community health. To achieve this, we seek outstanding individuals who share our values: compassionate, aspirational, responsive, and excellent.

We believe that a commitment to equity, diversity, and inclusion enhances patient care and fosters a happier workforce. We pride ourselves on being diverse and welcome applications from individuals of varied backgrounds, perspectives, and experiences.
