Long Term Conditions Clinical Coordinator – NCL Health Alliance

Full time; Fixed term

We are seeking an experienced, personable, and organized clinician to serve as maternity cover and lead a small team providing care coordination for individuals with multiple long-term conditions.

Salary: NHS Band 8a or equivalent

Closing date: 9 May 2025

The NCL Health Alliance is the all-in NHS provider collaborative for North Central London, working across primary care, acute, mental health, specialist, and community providers. It is part of UCLPartners, a health innovation partnership. This role is recruited on behalf of University College Hospital NHS Trust, with the postholder employed by UCLH.

This position is part of the Complex Long Term Conditions Service (CLTCS), an evaluated programme aimed at improving care coordination for people with complex long-term conditions within North Central London. Our goal is to develop and assess a new approach to managing multi-morbidity.

The LTC Clinical Coordinator will collaborate with early adopter sites and a small team to develop systems supporting remote review of high-risk and complex patients within the Primary Care Network (PCN).

We seek a dynamic clinician or individual with a clinical background—such as a nurse, allied health professional, physician’s associate, or mental health practitioner—experienced in primary care, community services, acute care, or mental health. Candidates should have experience working with long-term conditions and across professional and agency boundaries to enhance care.

This is a fixed-term maternity cover role until the end of March 2026, with potential for extension. Suitable candidates include Band 8a AHPs, nurses, clinical pharmacists, physician associates, or managers with a clinical/AHP background.
