Job summary
An exciting opportunity has arisen for an experienced Palliative Care CNS to work within the community setting. We are looking for an experienced, autonomous, and innovative nurse to join the integrated specialist palliative care nursing team.
This is a Band 7 community-based palliative care CNS post, working alongside Community Nursing Services, Consultants, Therapists, and the wider MDT to ensure that the service provides expert, high-quality, patient-focused care, advising and supporting patients, families, and Healthcare Professionals.
You will work closely with GPs, Community Teams, and Acute Trust staff. As an integral part of community nursing, you will support community teams to feel confident and competent in delivering good palliative and end-of-life patient-centered care and provide expertise for patients with palliative care needs across the borough.
Enthusiasm, motivation, compassion, and leadership are essential qualities for this specialist nurse role in a challenging environment. You should have a commitment to and a progressive approach within palliative care. You must demonstrate excellence in clinical practice, act as a resource for information and education, and work alongside other specialist services.
