Job summary

An exciting opportunity has arisen for an experienced Palliative Care CNS to work within the community setting. We are looking for an experienced, autonomous, and innovative nurse to join the integrated specialist palliative care nursing team.

This is a Band 7 community-based palliative care CNS post, working alongside Community Nursing Services, Consultants, Therapists, and the wider MDT to ensure that the service provides expert, high-quality, patient-focused care, advising and supporting patients, families, and Healthcare Professionals.

You will work closely with GPs, Community Teams, and Acute Trust staff. As an integral part of community nursing, you will support community teams to feel confident and competent in delivering good palliative and end-of-life patient-centered care and provide expertise for patients with palliative care needs across the borough.

Main duties of the job

Enthusiasm, motivation, compassion, and leadership are essential qualities for this specialist nurse role in a challenging environment. You should have a commitment to and a progressive approach within palliative care. You must demonstrate excellence in clinical practice, act as a resource for information and education, and work alongside other specialist services.

Education/Qualifications (Essential):

  • First-level nursing registration
  • First-level degree in related subject or evidence of work towards this
  • Teaching and assessing qualification or experience
  • Advanced Communication Skills Course
  • Level 2 psychology training

Experience (Essential):

  • Relevant post-registration experience at band 6 or above in this or an appropriate specialty
  • Experience in clinical and managerial responsibilities
  • Experience in teaching and developing materials
  • Well-developed clinical practice
  • Ability to develop new ways of working
  • Experience managing complex patient care, including symptom control and psychological support
  • Ability to manage own caseload
