Job summary

Thornbury Community Services, part of Acacium Group, is seeking a Clinical Lead to manage a caseload of adult and paediatric packages across Yorkshire. This permanent, full-time role includes on-call duties on a rota and field-based travel with a £49,000-£53,000 salary plus £3,000 car allowance.

Responsibilities

• Undertake initial holistic nursing assessments in partnership with the client, their family and the care team.
• Develop individualised care plans in partnership with the client and family; communicate with staff and ensure records are maintained.
• Identify and manage risks as part of care provision and lead investigations of clinical incidents with thorough documentation.
• Support staff by delivering client-specific clinical training, mentorship and supervision with feedback and performance reviews.

About the Role

In this role, you will:

• Undertake initial holistic nursing assessments in partnership with the client, their family and the care team.
• Develop individualised care plans in partnership with the client, their family and other healthcare professionals. Effectivelycommunicate with staff on the clinical condition/diagnosis and treatment/discharge plans of clients and ensure accurate records are maintained by staff.
• Identify and manage risks as part of care provision and lead the investigation and resolution of clinical incidents, ensuring timely and thorough documentation.
• Support staff by delivering client-specific clinical training, mentorship and supervision by providing feedback,conducting performance reviews, and supporting professional development.
