Job summary

Thornbury Community Services, part of Acacium Group, is hiring a RNLD Clinical Lead to manage complex community care packages in North Yorkshire. The role requires clinical leadership, safeguarding high-quality nursing assessments and care planning.

You will lead a field-based team, deliver training and supervision, and ensure thorough documentation while working closely with clients, families, and care staff. A valid NMC pin and driving license are essential.

Qualifications

  • Active NMC pin required for the role.
  • Experience leading community teams supporting learning disabilities, autism and mental health needs.
  • Must have a valid driver’s license and access to a vehicle.

Responsibilities

  • Undertake initial holistic nursing assessments and develop individualised care plans with clients and families.
  • Communicate with the care team about clinical condition, treatment or discharge plans and maintain accurate records.
  • Identify and manage risks, lead investigations of clinical incidents and ensure timely documentation.

We are hiring a RNLD Clinical Lead to join Thornbury Community Services who are an 'outstanding' CQC-rated care provider that deliver complex care at home or in the community.

In this role you will manage a caseload of packages to support individuals with learning disabilities, autism and mental health needs.

In this role, you will:
  • Undertake initial holistic nursing assessments and develop individualised care plans in partnership with the client, their family and the care team.
  • Effectivelycommunicate with the care team on the clinical condition or diagnosis and treatment or discharge plans of clients, ensuring accurate records are maintained.
  • Identify and manage risks as part of care provision and lead the investigation and resolution of clinical incidents, ensuring timely and thorough documentation.
  • Support the care team by delivering client-specific clinical training, mentorship and supervision by providing feedback,conducting performance reviews, and supporting professional development.

  • A learning disabilities nurse with an active NMC pin
