Thornbury Community Services, part of Acacium Group, is recruiting a Clinical Lead to manage adult and paediatric packages across Yorkshire. Field-based with frequent travel, you will oversee care delivery in the community and on-call rotations to ensure high-quality outcomes.

The position offers £49,000–£53,000 plus a £3,000 car allowance. A Paediatric nursing degree with an active NMC pin and community leadership experience are essential for this role.

Qualifications

About the Role

In this role, you will:

  • Undertake initial holistic nursing assessments in partnership with the client, their family and the care team.

  • Develop individualised care plans in partnership with the client, their family and other healthcare professionals. Effectivelycommunicate with staff on the clinical condition/diagnosis and treatment/discharge plans of clients and ensure accurate records are maintained by staff.

  • Identify and manage risks as part of care provision and lead the investigation and resolution of clinical incidents, ensuring timely and thorough documentation.

  • Support staff by delivering client-specific clinical training, mentorship and supervision by providing feedback,conducting performance reviews, and supporting professional development.
