Qualifications

• Experience in care, health and social care or a client-facing care coordination role.
• Ability to write clear, person-centred care plans and risk assessments.
• Strong communication and relationship-building skills.

Responsibilities

• Manage client enquiries and care consultations.
• Create and maintain person-centred care plans.
• Conduct quality assurance visits and service reviews.
• Build relationships with clients and community partners.
• Work closely with Care Professionals for effective delivery.

Skills

Experience in care coordination

Confidence in conducting care assessments

Understanding of CQC standards
