Symphony Healthcare Services Limited is seeking an experienced Long Term Conditions / Chronic Disease Nurse to join our team on a part-time basis (20 hours per week). The role involves assessing undifferentiated patients in face-to-face consultations and via telephone using digital triage platforms.

Qualifications

• Experience managing patients with chronic diseases.
• Ability to work autonomously within a multidisciplinary team.
• Knowledge of local and national health policy.

Responsabilités

• Manage a designated caseload of chronic patients at home or practice.
• Interest/specialism in Asthma, COPD and Diabetes advantageous.
• Mentor junior staff and support nursing services development.
• Maintain accurate patient records in line with policy and legislation.
• Deliver care according to NSF, NICE guidelines and evidence-based practice.
