Job summary

Symphony Healthcare Services Limited is seeking an experienced Long Term Conditions / Chronic Disease Nurse for a part-time role at our Somerset sites. The position is 20 hours per week with hybrid working where applicable.

You will provide patient care from history taking, clinical assessment and treatment, while applying independent professional judgment within NHS guidelines. Experience in one chronic disease area is desirable.

Qualifications

• Experienced NMC registered nurse.
• Desirable: experience in chronic disease areas (Asthma, COPD, Diabetes).
• Ability to work autonomously and as part of a multi-disciplinary team.
• Part-time hours: 20 per week; hybrid working where applicable.

Responsibilities

• Take patient history, perform clinical assessment, diagnose and plan treatment.
• Provide treatment and evaluate patient care.
• Make differential diagnoses using clinical judgment and problem solving.
