Job summary

Symphony Healthcare Services Limited is seeking an experienced Long Term Conditions / Chronic Disease Nurse for a part-time role at our Somerset sites. The position is 20 hours per week with hybrid working where applicable.

You will provide patient care from history taking, clinical assessment and treatment, while applying independent professional judgment within NHS guidelines. Experience in one chronic disease area is desirable.

Qualifications

• Experienced NMC registered nurse.
• Desirable: experience in chronic disease areas (Asthma, COPD, Diabetes).
• Ability to work autonomously and as part of a multi-disciplinary team.
• Part-time hours: 20 per week; hybrid working where applicable.

Job description

Symphony Healthcare Services is seeking an experienced Long Term Conditions / Chronic Disease Nurse, ideally with a specialism / interest in Asthma, COPD and Diabetes. You will become part of a dedicated team based at our Symphony South practices on a part time basis.

You will be an experienced, NMC registered Nurse who, whilst acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of patient care.

You will be able to demonstrate the ability to make differential diagnosis using decision making and problem solving skills whilst making professional autonomous decisions for which they are accountable. You will also possess the ability to work independently and collaboratively, as part of a multi-disciplinary team, with other health care professionals and disciplines, supporting the delivery of policy and procedures.

Experience in at least one chronic disease area is desirable.

This vacancy is a permanent part time contract of 20 hours per week.

The pay range for this role is £21.26 - £22.72 per hour.
