THE DOW SURGERY in Redditch is seeking a Chronic Disease trained Nurse to join the team for 30 hours per week (8 sessions). You will undertake nursing assessments and provide care alongside GPs, while mentoring Health Care Assistants and ensuring adherence to practice policies.

• Nurse with chronic disease care experience.
• Knowledge of the GMS Contract.
