• Qualified Social Worker with experience in pathway planning and reviews.
• Ability to conduct risk assessments and coordinate care for young people.

Responsibilities

• Support the team manager in progressing pathway planning for separated young people.
• Attend reflective group supervision and maintain records of visits.

Please be aware this is a Long Term Temporary post to start ASAP.

Candidate Requirements

• Social Work qualified
• Experience of Pathway planning and pathway plan review meetings
• Experience with keeping in touch visits and recording these

• Full driving license and access to a vehicle with business insurance
• Attendance at reflective group supervision
• Completion of risk assessments and coordination of the team around a care leaver
• Support the team manager in progressing pathway planning for separated young people
• Openness to monthly case supervision involving reflection
