Job summary

HCRG Care Group is seeking a Clinical Triage Practitioner to support Children’s Specialist Services across Bath & North East Somerset, Swindon and Wiltshire. The role involves triage of referrals, timely assessment and clear outcomes within a multidisciplinary team.

You will work in a hybrid model with travel across the BSW locality to support service delivery, ensuring coordinated care and safeguarding for children and young people.

Qualifications

• Registered professional with current registration (NMC/HCPC).
• Evidence of continuing professional development and clinical knowledge.
• Experience in child and family-centred care and safeguarding procedures.
• Minimum 3 years post-registration experience across relevant services.
• Ability to travel across Wiltshire, Bath & North East Somerset and Swindon.
• Experience collaborating with multiple agencies and teams.

Responsibilities

• Undertake clinical triage of referrals into Children's Specialist Services and School Aged Health Services.
• Support multidisciplinary triage for complex referrals with the Clinical Triage Lead.
• Provide specialist clinical advice to Care Coordination and Single Point of Access.
• Direct children and families to appropriate services via effective care navigation.
• Communicate complex information to families and professionals as required.
• Collaborate with health, education, social care and third-sector organisations.
• Identify safeguarding concerns and manage clinical risks and pathway issues.
• Contribute to service development, governance and SOP implementation.
