HCRG Care Group is seeking a Clinical Triage Practitioner to support Children’s Specialist Services across Bath & North East Somerset, Swindon and Wiltshire. The role involves triage of referrals, timely assessment and clear outcomes within a multidisciplinary team.

Clinical Triage Practitioner
An exciting opportunity has arisen for a Clinical Triage Practitioner to support the delivery of Children's Specialist Services across Bath & North East Somerset, Swindon and Wiltshire. The post holder will undertake clinical triage of referrals, including urgent referrals, ensuring timely assessment, decision-making and communication of outcomes. Working as part of a multidisciplinary team, the successful candidate will support complex referral discussions, provide clinical advice and contribute to effective care coordination across services.
The role requires close partnership working with healthcare professionals, education services, voluntary sector organisations and families to ensure children and young people receive appropriate, safe and effective support through a coordinated and integrated care pathway.
To be agreed within Bath & North East Somerset, Swindon or Wiltshire, with hybrid home working available. Travel across the BSW locality may be required to support service delivery and partnership working.
37.5 hours per week.
