Job summary

NHS Central East ICB is seeking a Band 6 CHC Assessor to perform high-quality Continuing Healthcare assessments in line with the National Framework. You will build evidence-based recommendations and support MDT decisions while managing a defined caseload with strong governance and documentation standards.

The role requires nursing or allied health registration, degree-level education, and NHS integration knowledge.

Qualifications

• Relevant qualification or equivalent experience in health, social care, business administration, or a related field
• Educated to degree level or equivalent experience in a relevant field (e.g., health, care, public services, management, or related discipline).
• NMC Professional Registration

Responsibilities

• Undertake comprehensive CHC assessments in line with the National Framework, completing high‑quality Decision Support Tools and recommendations.
• Gather and analyse relevant evidence from health and social care records, professionals, individuals, and families to support accurate eligibility considerations.
• Manage a defined caseload, ensuring assessments, reviews, and documentation are completed within required timescales.
• Participate in MDT meetings, contributing clinical insight and ensuring assessments are person‑centred and well‑coordinated.
• Communicate sensitively and professionally with individuals, families, and representatives, explaining the CHC process and addressing questions where appropriate.
