Job summary

Qualifications

• Registered nurse with NMC registration.
• Experience in Continuing Healthcare pathways is desirable.
• Ability to work within an MDT and with social care partners.

Responsibilities

• Deliver end-to-end CHC pathway assessments and reviews.
• Carry out eligibility assessments and case management.
• Work with families, local authorities and MDT colleagues to secure appropriate care.
