Job summary

The Dudley Group is seeking a Band 6 nurse to assess individuals in community, home, or hospital settings to determine eligibility for NHS Continuing Healthcare or Funding Nursing Care. The role involves MDT coordination and ensuring timely, guideline-compliant reviews.

The post holder will work as part of the MDT, explaining processes to patients and families, and supporting a transparent funding pathway within NHS frameworks.

Qualifications

• Highly skilled nurse competent to practice within the extended role.
• Evidence of on-going post-basic qualifications in area identified.
• Educated to degree Level /or Equivalent (or working towards this)
• Evidence of commitment to lifelong academic learning

Responsibilities

• Assess individuals in the community/home/hospitals to determine NHS Continuing Healthcare eligibility.
• Coordinate the assessment with the multidisciplinary team (MDT) and determine eligibility.
• Explain the process to patients and families, health and social care colleagues, and care providers.
• Ensure the review process for NHS Funded Care is undertaken in line with current guidance.
