Job summary

Central East Integrated Care Board seeks a CHC Assessor (Band 6) to deliver high-quality Continuing Healthcare assessments across the Ely area. You will gather information, analyse needs, and produce Decision Support Tools for multidisciplinary teams with empathy and clinical judgment.

The role requires strong assessment skills, governance awareness, and ability to manage a defined caseload while maintaining timely documentation.

Qualifications

• Qualified in health, social care, or related field with degree level (or equivalent).
• NMC registration is required.
• Experience coordinating projects or service delivery in health/care settings.
• Knowledge of NHS structures and governance processes.

Responsibilities

• Undertake CHC assessments in line with the National Framework and complete high-quality Decision Support Tools and recommendations.
• Gather and analyse evidence from health and social care records, professionals, individuals, and families.
• Manage a defined caseload and ensure assessments and documentation are completed within timescales.
• Participate in MDT meetings, contributing clinical insight and ensuring person-centred assessment processes.
• Escalate complex cases for senior review and ensure governance and data protection requirements are met.
