Job summary

NHS Humber and North Yorkshire Integrated Care Board is seeking a Band 6 Case Manager and Assessor to lead end-to-end CHC processes, including eligibility assessment, reviews and care package planning for adults with complex health needs across the Humber and North Yorkshire footprint.

You will work with an integrated team of clinicians, social workers and admin staff, liaising with families, local authorities and MDTs to ensure timely, high-quality assessments and robust documentation in line

Qualifications

• Registered Nurse with post-qualification experience
• Evidence of ongoing professional development
• Knowledge of CHC & FNC criteria and NHS/NHS social care framework
• Understanding of Mental Health Act, Mental Capacity Act and safeguarding policies
• Ability to produce clear, high-quality documentation and assessments
• Experience in multi-disciplinary/multi-agency working
• Willingness to travel across the county daily
• Strong communication and negotiation skills

Responsibilities

• Undertake comprehensive CHC assessments, eligibility and reviews
• Develop and monitor care packages with NHS and Local Authority partners
• Collaborate with MDTs, families and carers to implement care plans
• Maintain accurate documentation and support data processing and payments
• Provide clinical advice and support to colleagues and other agencies
• Ensure safeguarding and quality governance processes are followed
