Job summary

Herefordshire Council is seeking a CHAT Advisor in the Children with Disabilities Team in Hereford. The role involves contacting families, assessing needs, and developing support plans to improve wellbeing and inclusion.

Qualifications

• Ability to listen to a family’s needs and consider support options.
• Good attention to detail with IT skills.
• Ability to work in a fast-paced environment within a friendly team.

Responsibilities

• Provide information, advice, guidance and support to children and young people with disabilities and their families, ensuring access to the right services at the right time.
• Make initial contact with families following referrals, discuss needs over the phone and other appropriate channels, and develop outcomes and support plans.
• Act as a point of contact for families awaiting specialist services, offering information, reassurance and signposting to community, health, education and voluntary sector support.
