Job summary

Herts at Home Limited is seeking a dedicated Care Coordinator (Fixed Term - 12 Months) to ensure safe, person-centred care for service users in North Herts. You will manage care visits, rota planning, and act as the link between care teams, families, and healthcare professionals.

The role requires strong organisational and communication skills, the ability to handle on-call duties, and a compassionate, proactive approach to delivering high-quality community care.

Qualifications

• Experience in customer service, care coordination, scheduling or call handling.
• Experience planning workloads and creating staff rotas in a busy environment.
• Excellent organisational skills with the ability to prioritise and work under pressure.
• Strong IT skills including Microsoft Office; Nourish or similar software is advantageous.

Responsibilities

• Plan, coordinate and schedule care services to ensure continuity and high standards of care.
• Create and maintain staff rotas using the electronic scheduling system and ensure timely delivery of schedules.
• Monitor care delivery, respond to changes in service user needs and staffing requirements.
• Produce reports and analyse scheduling data to support performance and compliance.
• Provide hands-on care support when required and participate in the on-call rota.
• Liaise with external agencies regarding new care packages and changes in requirements.
