DePaul is seeking a dedicated Community Outreach Case Worker to support people experiencing homelessness in Derry/Londonderry. You will deliver person-centred assistance, carry out risk assessments and develop support plans with service users, linking with partner agencies to coordinate care.

Your role requires a Level 3 social care qualification, experience with vulnerable women, and strong IT and reporting skills to maintain records and monitor progress within a harm-reduction framework.

Qualifications

• A formal Level 3 qualification in a social care field.
• At least 2 years paid experience working with vulnerable women, desirably leaving prison experience; or 3 years with such experience.
• 3 formal qualifications at Level 2 to include Maths and English (GCSEs or NVQ)

Responsabilités

• Provide service user support in a person-centred, professional manner.
• Assess safety and wellbeing needs and develop plans.
• Develop support plans with service users and liaise with external services.
• Advocate on behalf of service users to achieve outcomes.
• Maintain accurate written and electronic records and reports
