Job summary

Hampshire County Council is seeking a CART Case Worker for the Public Service Plaza in Havant. The role operates on a hybrid basis, with 37 hours per week, permanent contract, and a salary of £28,697 - £31,573 per annum.

You will respond to enquiries, signpost resources, and support adults’ health and care services while maintaining compassionate communication. Join a team handling around 100,000 enquiries annually, contributing to outcomes under the Care Act (2014) and progressing professional

Qualifications

• Confident telephone communication and the ability to ‘type and talk’.
• Sound IT and record-keeping skills.
• Flexibility and adaptability.
• Commitment to demonstrating values with compassion, empathy and respect.
• Awareness of the Care Act (2014) or willingness to research ahead of the interview.

Responsibilities

• Respond to enquiries by telephone, online referrals and web chat, and know when to elevate cases to management.
• Complete wellbeing checks using a strengths-based approach and assess needs for occupational therapy equipment services.
• Provide signposting and direct individuals to the relevant team or partner agency.
• Meet performance targets for calls, online referrals and web chat and ensure quality of responses.
