Job summary

Connect2Hampshire is recruiting a Case Worker for Hampshire County Council's CART Team in Havant. You will assess residents' needs and provide support to maximise wellbeing, using strengths-based practice to connect people with community resources.

You will train for up to 4 weeks before utilising a hybrid model (1 day in the office, 4 days at home). You will handle enquiries by phone, online forms and web chat, working with social/health care professionals to promote independence while

Responsibilities

• Support individuals by completing assessments and reviews to promote well-being and independence.
• Provide information and advice in accessible formats to meet individual needs.
• Identify rising risk in cases and respond appropriately, seeking advice when needed.
• Recognise safeguarding situations and know how to report them.
• Manage workload effectively and prioritise appropriately.
• Build positive working relationships with multi-disciplinary colleagues.
• Prepare for and participate in supervision with a senior team member.
• Support colleagues in achieving the team's operational priorities.
