Position: Care Practitioner at Woodlands Neurological Rehabilitation Centre, York
The role involves ensuring the safe operation of shifts, working with the nurse in charge, engaging in care planning, managing medications, supporting the MDT with patient timetables, maintaining the environment, and participating in audits and change implementation.
Working Hours and Salary
- 36 hours per week
- Days, Nights, Weekends as per rota
- Salary: £14.93 per hour
Key Responsibilities
- Provide quality care and support patients through their care pathway, promoting independence, dignity, privacy, and safety.
- Assist qualified staff and healthcare professionals in assessing, planning, implementing, and evaluating care without direct supervision.
- Lead holistic care delivery as a Rehabilitation Team Leader, ensuring standards are met and improved.
- Communicate effectively within the team, with other healthcare professionals, patients, families, and visitors.
- Maintain a safe, respectful, and inclusive environment.
- Assess, evaluate, and report on actions and activities to registered practitioners or the multidisciplinary team.
- Supervise and delegate tasks to junior team members, assessing their skills and competencies.
- Undergo training and achieve competencies for medication management and other service-specific skills.
- Conduct supervisions and act as a preceptor for new staff, providing guidance and support.
- Support the creation of care plans, case reviews, and discharge notes tailored to service users' needs.
Essential Qualifications and Experience
- Educated to NVQ Level 3/4, foundation degree, or equivalent through training or relevant experience.
Desirable Skills and Qualifications
- Experience in the specialty area.
- Grade C or above in GCSE Math and English (or equivalent).
- Willingness to undertake training to teach in the lifelong learning sector.
