Care Practitioner at Woodlands Neurological Rehabilitation Centre in York

The purpose of the role is to ensure the safe running of the shift, working in partnership with the nurse in charge, engaging with care planning, managing the administration, storage and ordering of medications, supporting the MDT with the patients' personalized timetabling, ensuring the environment is well maintained. To participate in auditing and implementation of change.

  • Salary: £14.93 per hour

What you'll be doing:

  • Ensure quality care and rehabilitation services that support patients throughout their care pathway, promoting independence and safeguarding their dignity, privacy, and safety.
  • Support qualified staff and healthcare professionals in assessment, planning, implementation, and evaluation of care without direct supervision of a registered practitioner.
  • Act as a Rehabilitation Team Leader to ensure holistic care delivery and standards enhancement.
  • Communicate effectively within the team, with other healthcare professionals, patients, families, and visitors.
  • Maintain high standards in a safe environment that promotes equality and sensitivity for all individuals.
  • Regularly assess, evaluate, and report on actions and activities to registered practitioners and/or the multidisciplinary care team.
  • Supervise and delegate work to junior team members, assessing their skills, competencies, and qualifications, including NVQs.
  • Undergo training and develop competencies for the safe administration, storage, and ordering of medications.
  • Participate in training to develop skills to meet service user needs.
  • Conduct supervisions with junior staff to ensure competencies, expectations, and standards are maintained.
  • Act as a preceptor for new staff, providing guidance and support.
  • Support the creation of care plans, case reviews, and discharge notes tailored to service users' needs.

What you'll have:

Essential

  • Educated to NVQ 3/4, foundation degree plus additional training to diploma level or equivalent through experience or short courses.

Desirable

  • Experience in the specialty area.
  • Grade C or above in GCSE Math and English (or equivalent).
  • Willingness to undertake 'Preparing to Teach in the Lifelong Learning Sector' training.
