Job summary

Inclusia Living is seeking a Care Planner in Stoke-on-Trent to develop person-centred, measurable support plans in collaboration with service users, families and commissioning partners.

The role is contract-based and designed for an experienced professional to help shape a new service model from the ground up, with a focus on compliant documentation and high-quality planning processes.

Qualifications

  • Experience in care planning, case management or related assessment role.
  • Strong knowledge of person-centred planning approaches and relevant legislation.
  • Experience working with local authority commissioners and multi-agency teams.
  • Excellent written skills—ability to write plans that are clear and professionally presented.
  • NVQ/QCF Level 4 or above, or equivalent qualification is desirable.
  • Enhanced DBS check is required.

About The Role

As a Care Planner you'll work closely with people who use our services, their families and our commissioning partners to develop support plans that are genuinely person-centred, measurable and compliant. This is a contract role suited to an experienced professional who wants to contribute to a new service model from the ground up.
