Complex Care Professionals Ltd is seeking a proactive Care Coordinator to support high-quality, person-centred care and help grow services in the North West. The role requires strong communication, rota management, and the ability to build relationships with clients, staff and healthcare professionals.

Qualifications

• Experience in a domiciliary care coordination role (preferred).
• Strong organisational and problem-solving skills.
• Confident communicator with a proactive approach.
• Experience with rostering systems (e.g. Access Planner desirable).
• Able to manage multiple priorities in a fast-paced environment.
• Full UK driving licence (preferred).

Responsibilities

• Coordinate care packages from referral through to delivery.
• Manage and optimise staff rotas (holidays, absence, coverage).
• Recruit, onboard, and support care staff.
• Liaise with clients, carers, and professionals (GPs, Commissioners, Nurses).
• Maintain high standards of care, compliance, and service quality.
• Conduct staff supervisions and support performance management.
• Support business growth through client relationship management.
• Participate in an on-call rota.
