Job summary

Wirral Council is seeking a Placement & Brokerage Officer to coordinate timely and effective care packages for adults with assessed needs. You will support safe hospital discharges, broker care that enables people to return home with the right support, and work closely with care providers to match individuals to services that meet their needs.

You will be highly organised, proactive and confident communicating with professionals, providers and stakeholders.

Qualifications

• Must be highly organised, proactive and able to communicate with professionals, providers and stakeholders.
• Strong IT skills and a keen eye for detail are required.
• Ability to juggle competing priorities with calm and professionalism.
