Zusammenfassung

Cheshire East Council’s Care Brokerage Team is recruiting a Brokerage Officer in a full-time role (37 hours per week, Monday to Friday). You will manage referrals for Adult Social Care and Health, build provider relationships, and ensure timely hospital discharges to keep care pathways flowing.

The successful candidate will have Care Sector experience, strong relationship skills, and IT proficiency, within a dynamic, high-pressure environment. Cheshire East Council values wellbeing and inclusion.

Qualifikationen

• Experience of working within the Care Sector.
• Excellent relationship management skills.
• Ability to work under minimal supervision in a fast paced environment.
• Excellent IT skills.

Aufgaben

• Manage referrals for Adult Social Care and Health services via a single point of access.
• Develop and maintain provider relationships to match clients to services.
• Raise awareness of referral processes with providers and referral sources.
• Identify and refer to appropriate providers based on referral information.
• Monitor progress of referrals and update systems.
• Facilitate timely hospital discharges to support system flow.
