Job summary

The NHS Jobs team in Gloucester is seeking a Patient Care Coordinator to support a Primary Care Network across multiple surgeries. You will coordinate and navigate access to health and social care services, helping patients, carers and families to receive the right care at the right time.

This non-clinical role focuses on improving quality of life, with training provided in care coordination, patient navigation and relevant IT systems.

Responsibilities

• Work collaboratively with patients, families and carers to improve understanding of the individual's condition and support the development and regular review of personalised care and support plans to help manage their needs and achieve better health outcomes.
• Respond to queries, arrange and manage appointments, and provide high quality written and verbal information to support informed decision making.
• Identify unmet needs and refer patients to relevant services, including social prescribing link workers, community groups, mental health services, and voluntary sector organisations.
• Maintain accurate, up-to-date records of patient interactions and interventions using relevant clinical systems (e.g. SystmOne).
• Identify and support unpaid carers, helping them access appropriate services and resources.
