Arc Bucks Primary Care Network is recruiting a Care Coordinator to provide personalised care and support for patients across four GP practices in South Buckinghamshire. You will work with care homes, carers, and multidisciplinary teams to ensure joined-up care and timely information for patients and families.

In this role you will manage appointments, signpost services, support shared decision-making, and maintain accurate records while upholding patient confidentiality.

Formación

• Essential experience in care co-ordination, healthcare, social care or patient-focused role.
• Excellent communication and interpersonal skills with a compassionate, person-centred approach.
• Strong organisational skills with ability to manage competing priorities and follow actions through to completion.
• Confidence working with patients, carers and professionals across teams/organisations.
• Ability to work independently while contributing to a multidisciplinary team.

Responsabilidades

• Personalised care and patient support with proactive population health identification.
• Act as a point of contact and information resource for patients, families and carers with empathy and signposting.
• Prepare patients for shared decision-making and help create personalised care plans.
• Coordinate and navigate care across health and care services in collaboration with Social Prescribing Link Workers and Health & Wellbeing Coaches.
• Coordinate MDT meetings, diary management, and prepare reports for Arc Bucks PCN.
