Job summary

Service Care is seeking a Care Navigator to support GP practices and Care Hubs across Sefton and the wider Liverpool area. This contract position offers flexible hours, part-time or full-time, with a competitive pay rate of £17 per hour (Ltd).

The role focuses on connecting patients with GPs, guiding them through services and maintaining clear records. Ideal candidates have experience in primary care settings, strong communication, and ability to work within a healthcare team.

Qualifications

• Previous experience in a Care Navigator or Receptionist role in a primary care setting.
• Knowledge of EMIS (preferred)
• Excellent communication and interpersonal skills
• Strong organisational and administrative abilities
• A patient-focused approach with professionalism in handling enquiries
• Ability to work effectively within a healthcare team

Responsibilities

• Supporting GP practices and our client Care Hubs with administrative duties
• Handling patient enquiries with professionalism and compassion
• Assisting patients in navigating healthcare services and procedures
• Maintaining accurate patient records using the EMIS clinical system (desirable experience)
• Providing information and guidance to patients about healthcare options
• Working closely with GPs and healthcare teams to ensure effective patient care and smooth operations
• Supporting day-to-day administrative functions to improve patient flow and service delivery
