A leading GP provider network in Central London is seeking a Care Navigator to help individuals experiencing homelessness. This role involves assessing eligibility for housing, managing a housing patch, and offering advice on tenancy. The ideal candidate will have experience in housing and a passion for helping vulnerable populations while being part of a supportive team.

Healthcare Central London Ltd (HCL) is the GP Federation owned by 33 General Practices covering the Central London (Westminster) area. We support 4 Primary Care Networks (PCNs) and operate several NHS contracts including Community Dermatology, Community Cardiology, Secondary Care Referral Service, and out-of-hospital provisions subcontracted to our practices.
We are the host employers for a growing team of ARRS roles including Clinical Pharmacists, Pharmacy Technicians, First Contact Physiotherapists, Dieticians, Paramedics, Social Prescribers, Care Coordinators, Digital & Transformation Leads, Nursing Associates, GPAs, and Care Co-ordinators.

Given the increase in rough sleeping in Westminster, this role supports individuals experiencing homelessness, helping them access housing and services, and improve their life skills and well-being. Responsibilities include assessing clients' eligibility, managing a designated housing patch, and providing advice on tenancy matters, welfare benefits, and re-housing requests. Experience in supported housing is useful but not essential.
The role involves working face-to-face with homeless and asylum-seeking patients at least once weekly, primarily in surgery or hotel settings. Candidates should have experience in general needs housing and tenancy management, with a strong desire to help people maintain independence.
The successful candidate must be proficient in written communication and join our Care Navigator Team, supporting patients, especially those with long-term conditions, through care coordination and navigation within GP practices. The role is non-patient facing, with support provided via telephone, and involves working across several practices within a PCN.
