We are seeking committed and diligent staff to work as Network Navigators in our three Acute Mental Health Wards at Blossom Court, St Ann's Hospital. The roles are full time and in person with face-to-face working as a minimum expectation. These are new roles to work alongside our Patient Flow service and will be part of a wider system of supporting and managing patient access and flow within the North London Mental Health Partnership.
Reporting directly to the Discharge Coordinator, you will be assigned to an acute ward and will operate within the ward daily, ensuring 72-hour formulation meetings are in place, liaising with ward staff, and ensuring all actions are completed to facilitate the safe discharge of in-patients when clinically ready.
Hours of work are Monday to Friday, 9 am to 5 pm. Professional registration is not required for these roles.
The partnership between Barnet, Enfield and Haringey Mental Health NHS Trust (BEH) and Camden and Islington NHS Foundation Trust (C&I) has been strengthening since its establishment in 2021, forming the North London Mental Health Partnership.
You may be required to work at different locations under the Trusts to meet service needs. The Trust reserves the right to assign staff to various locations as necessary, temporarily or permanently.
Network Navigators will ensure each patient’s care network is involved early in admission to inform care planning and facilitate timely discharge. They will organize this system around the patient, joining key meetings during admission.
Their approach will be therapeutic, strengths-based, and recovery-oriented, involving a broad range of relevant individuals and services, including carers, community mental health and substance misuse teams, voluntary organizations, criminal justice staff, and housing providers.
They will work closely with Discharge Coordinators and ward teams to identify and address barriers to discharge early, ensuring patients are discharged when clinically ready and facilitating carer contributions.
