Sign-on Bonus: For a limited time, MaineHealth is offering up to $10,000 for eligible experienced Registered Nurses with 1-2 years of experience and $20,000 for those with more than 2 years of experience!
Position Purpose: To support the interdisciplinary team in facilitating patient care, improving clinical outcomes and patient satisfaction, managing care costs, and providing timely information to payors. The role involves coordinating utilization management, care facilitation, and discharge planning under general supervision, accountable for a designated patient caseload, and planning effectively to meet patient needs and promote resource efficiency. Responsibilities include:
