Cornwall Partnership NHS Foundation Trust in Bodmin is seeking an Administrative Band 3 to support the North & East Integrated Transfer of Care Hub (iToCH). The role focuses on processing referrals, maintaining patient flow and coordinating with multi-agency partners. Part-time hours (30) over seven days, with a pro rata salary.

You will review blockages in patient flow, provide timely feedback to referrers and work within a multi-professional team to ensure smooth operation of the hub.

Qualifications

• NVQ 3 Administration or equivalent.
• GCSE English & Maths or equivalent.
• Computer literate with an excellent working knowledge of Microsoft Office, Excel and PowerPoint.

Responsabilités

• Process and manage the flow of patient referrals through the Transfer of care hub and local point of access.
• Monitor referrals on the single electronic referral system and triage in partnership with the multi-professional team.
• Review key blockages to patient flow and provide accurate feedback to referrers; liaising with multi-agencies for coordination of patient flow.
• Work as part of the Admin Support Worker team in a flexible, supportive fashion, including bank holidays and weekends.
• Prioritise workload effectively and work under pressure in a healthcare office environment.
