Job summary

Cornwall Partnership NHS Foundation Trust is recruiting a Band 6 Clinical Prioritiser for the North & East IToCH. The role is 30 hours per week, with long days, weekends and bank holidays. The post is pro rata for annual salary £39,959–£48,117, offering community-based care across Cornwall with frequent travel.

The successful candidate will triage and prioritise patient needs using RiO/STRATA, contribute to integrated care plans, and work within a multidisciplinary team to improve local services.

Qualifications

• NMC registration or AHP registration.
• Clinical knowledge and experience in a community setting.
• Awareness of national and local health care policies.
• Professional nursing or AHP qualification at degree level.
• Evidence of relevant postgraduate study or substantial experience in a health care setting.
• Teaching or mentoring qualification.

Responsibilities

• Coordinate demand and capacity to triage and prioritise care.
• Utilise RiO and STRATA to triage referrals.
• Support frailty management and intermediate care pathways.
• Provide advanced assessment and care planning for complex conditions.
