Job summary

Hastings & Rother Healthcare is seeking a full-time Care Coordinator to join our growing team in Hastings. You will coordinate care for Learning Disabilities, frail and palliative patients, while supporting safeguarding and MDT activities.

The role requires experience in General Practice or community healthcare, with the ability to lead projects and work both in-person and virtually in ward rounds and home settings. HRH offers a supportive, expanding environment.

Qualifications

• Experience in General Practice or community healthcare settings.
• Ability to coordinate care for Learning Disabilities and frail/palliative patients.
• Experience with safeguarding coordination and statutory obligations.
• Proactive team player who can lead small projects.

Responsibilities

• Coordinate care for LD, frail and palliative patients.
• Support MDT collaboration and coordinate ward rounds (virtual or in-person).
• Document and prepare support for patients in care, nursing and LD homes.
• Maintain safeguarding registers and conduct safeguarding searches.
• Work within the PCN MDT to coordinate care as directed.
