Job summary

Hastings & Rother Healthcare is seeking a full-time Care Coordinator to join the Enhanced Care Team. The role focuses on coordinating care for people with learning disabilities, frailty and palliative needs, including virtual and in-person ward rounds in care and nursing homes.

You will maintain safeguarding registers, work with the safeguarding team, and use EMIS for record-keeping. Strong IT literacy and teamwork are essential.

Qualifications

• Minimum GCSE grade C in Maths and English.
• Previous experience in General Practice or community healthcare settings.
• Competence with EMIS and administrative tasks, including record-keeping and minute-taking.

Responsibilities

• Coordinate care for patients with learning disabilities, frailty and palliative needs.
• Manage safeguarding concerns and maintain safeguarding registers and searches.
• Work with the safeguarding team to meet statutory safeguarding obligations.
• Coordinate, prepare and document support for patients in care, nursing and learning disability homes.
• Take part in virtual and in-person ward rounds.
• Facilitate multidisciplinary team collaboration to improve outcomes for vulnerable adults and families.
• Provide care coordination as part of the PCN multidisciplinary team, as directed by the PCN Clinical Director.
• Lead allocated projects or services, supporting the team to implement, monitor and review them.
