Job summary

Collens Healthcare Ltd is seeking a dedicated Care Coordinator to join our team in the UK. You will plan, develop, and maintain person-centered care services, and manage staff rotas to ensure efficient delivery of homecare with a focus on dignity and independence.

This role requires previous care experience, medication support, and strong IT skills. You will participate in on-call duties and contribute to GDPR-compliant record-keeping and reporting.

Qualifications

• Previous experience within a care setting.
• Experience administering medication and supporting individuals with care needs.
• Knowledge of risk assessments, care planning, and person-centred support.
• Good IT skills, including Microsoft Office.
• Excellent verbal and written communication skills.
• Strong organisational and problem-solving abilities.
• Level 3 Diploma in Care from a Skills for Care-endorsed trainer.

Responsibilities

• Plan, develop, and maintain person-centred, outcome-focused care services.
• Create and manage care staff rotas to ensure efficient service delivery.
• Monitor recruitment requirements and provide regular updates to the Registered Manager.
• Ensure services are delivered in line with regulatory requirements, CQC standards, and company policies.
• Build positive relationships with service users, families, professionals, and partner agencies.
• Maintain accurate electronic and written records in line with GDPR requirements.
• Participate in the on-call rota and provide direct care support when necessary.
• Respond appropriately to concerns, incidents, and complaints.
• Produce management reports and operational information as required.
• Develop links with community organisations to enhance service provision.
