Job summary

Autumn Search Limited partners with a growing Home Care provider in Woking, Surrey to recruit a caring, organised Care Coordinator. This is a frontline role with scope to take on more responsibility as the team expands.

You will help coordinate day-to-day care delivery, draft rotas, arrange cover, support safeguarding, and liaise with Social Workers, GP practices and NHS teams. Mon–Fri 9am–5pm with on-call rota duties.

Qualifications

• Experience as a Care Coordinator, Scheduler, Senior Carer, or Care Team Leader.
• Familiar with rotas, scheduling, and coordinating care staff.
• Ability to build relationships with clients, families and professionals.

Responsibilities

• Assist with preparing rotas and scheduling in daily care delivery.
• Coordinate visits and arrange cover when required.
• Support safeguarding procedures and client well-being.
• Liaise with Social Workers, GP practices and NHS teams.
• Provide high-quality care in the community during on-call rota shifts.

We are working with an established and growing Home Care provider in Woking, Surrey, who is looking for a caring, organised and motivated individual to join their team as a Care Coordinator.

What You'll Be Doing

Working alongside the Registered Care Manager, you'll help coordinate the day-to-day delivery of care, including:

• Assisting with preparing rotas and scheduling, including arranging cover when required
• Helping to ensure the well being of both clients and care staff
• Supporting safeguarding procedures
• Building positive relationships with clients, families and care workers
• Liaising with Social Workers, GP practices, NHS Discharge Teams and other professionals
• Support the Registered Care Manager with administration and day-to-day coordination
• Providing personal care in the community when required
• Participating in an additional paid weekday & 1-in-4 weekend on-call rota
