Job summary

Caremark in Medway is seeking a dedicated Care Coordinator to allocate client visits, recruit suitable carers and maintain office procedures. The role requires a UK driving licence and access to a car, supporting our local Dartford, Gravesham and Medway services.

You will coordinate rotas, onboarding, and liaise with clients, families and staff to ensure high-quality home care. The role is based in our Medway office and is full time, Monday to Friday, 9am–5:30pm.

Qualifications

Excellent communication skills with the ability to sell the service on the phone.
Fast learner with good organisational abilities.
Proficient IT skills; rotas and care plans are updated using our IT systems.
Team player who works well in a busy office.

Responsibilities

Organise rotas by liaising with the care team.
Manage new customer referrals, follow up enquiries and convert into care packages.
Onboard new customers and ensure care setup runs smoothly.
Liaise with customers, families and staff to meet care needs and respond to changes.
Maintain accurate care records and keep information up to date.
Provide exceptional customer service to clients and care teams.
Assist with on-call duties.

What the Care Coordinator role involves?

Organisation of rotas by liaising with our care team
Managing new customer referrals, following up enquiries and converting opportunities into new care packages
Onboarding new customers and ensuring care is set up smoothly from the start
Liaising with customers, families and care staff to ensure care needs are met and changes are responded to promptly
Maintaining accurate care records and ensuring information is kept up to date
Providing exceptional customer service for clients and care teams
Assist with on call duties
