Job summary

Suffolk Family Carers Ltd is seeking a proactive Care Coordinator to join the Care Management Service in Suffolk. You will coordinate referrals, work with health, social care and voluntary partners to ensure joined-up, person-centred care planning, and maintain accurate records.

• Excellent communication and relationship-building skills.

• Confident using digital systems and Microsoft packages; awareness of clinical/care systems (e.g., SystmOne) is desirable.

We're looking for a proactive, organised and compassionate Care Coordinator to join the Care Management Service in Suffolk. This is a rewarding opportunity to coordinate support for individuals and families, working closely with multidisciplinary services to reduce duplication, improve communication and help people access timely, person-centred care.

• Contract Terms: 1 year fixed term
• Interview date: Tuesday, 03 November 2026
• 1 x 37 hours - Ipswich West
• 1 x 37 hours - Bury St. Edmunds
• 1x 37 hours - Ipswich East & South Rural
• 1 x 37 hours - Central Suffolk, Mildenhall, Brandon, Newmarket

What you'll be doing as a Care Coordinator
