Job summary

Suffolk Family Carers Ltd is seeking a proactive Care Coordinator to join the Care Management Service in Suffolk. You will coordinate referrals, work with health, social care and voluntary partners to ensure joined-up, person-centred care planning, and maintain accurate records.

Qualifications

• Excellent communication and relationship-building skills.
• Ability to work positively with patients, carers, professionals and community partners.
• Well-organised and resilient team member who can prioritise a varied workload, work under pressure and stay calm in complex or sensitive situations.
• Confident using digital systems and Microsoft packages; awareness of clinical/care systems (e.g., SystmOne) is desirable.

Responsibilities

• Coordinating referrals into the Care Management Service and ensuring people receive the right support from the right service at the right time.
• Working with health, social care, voluntary and community partners to ensure joined-up, efficient and person-centred care planning.
• Maintaining accurate records, signposting to resources, supporting multidisciplinary meetings and liaising with patients, families and carers by phone, in clinics or during home visits as required.
