Job summary

Safi Homecare, a nurse-led domiciliary care provider in Leicestershire, is seeking an experienced Care Coordinator. You’ll link service users, carers, and local authorities, creating efficient rotas and person-centred care plans. The role offers progression toward a Care Manager position with training and support.

You’ll manage assessments, care plans, and liaison with the council brokerage, ensuring continuity and high-quality care across packages. A driving licence is essential.

Qualifications

  • Experience as Care Coordinator, Scheduler or Field Care Supervisor in domiciliary care.
  • Level 3 Diploma in Health and Social Care (or equivalent).
  • Willingness to work towards Level 5 Diploma and Care Manager role with support.
  • Direct experience delivering care under a local authority framework, ideally Leicestershire County Council Home Care.
  • Ability to write initial assessments, care plans and risk assessments.
  • Established relationships with council brokerage and social work teams.
  • Proficiency with electronic care management/rostering systems (CareLineLive/Access/Nourish/Birdie/PASS).
  • Driving licence and vehicle access.
  • Strong written English.

Responsibilities

  • Build and maintain staff rotas across geography, matching carers to service users.
  • Manage daily changes: sickness, leave, hospital admissions and new packages.
  • Carry out initial home assessments and produce person-centred care plans and risk assessments.
  • Be the main contact for Leicestershire County Council brokerage, manage offers and capacity.
  • Maintain records on the care management system (plans, meds, visits, alerts).
  • Monitor missed/late calls and escalate clinical concerns to the Registered Manager.
  • Review and update care packages and arrange reviews as needs change.
  • Support carers with spot checks, supervision and guidance.
  • Handle enquiries from service users, families, social workers and hospital teams.
  • Contribute to audits, quality assurance and CQC evidence, and participate in on-call rota.

Are you an experienced Care Coordinator who wants more scope, more responsibility and a clear route into management?

Safi Homecare is a nurse-led, CQC-registered domiciliary care provider and an approved provider on the Leicestershire County Council Home Care framework. We are recruiting a Care Coordinator to help us deliver and grow our community services across Leicestershire.

This is a role with real progression attached. Our Registered Manager is an Advanced Nurse Practitioner, and we are looking for someone who wants to develop towards a Care Manager position over time, with the training and support to get there.

What you will be doing

You will be at the centre of how our service runs day to day, holding the link between the people we support, our care staff and the local authority.

  • Building and maintaining efficient, geographically sensible staff rotas, matching carers to service users on skills, continuity, language and location
  • Managing the daily changes sickness, annual leave, hospital admissions, new packages and keeping rounds covered without breaking continuity
  • Carrying out initial assessments in people's homes and producing person-centred care and support plans and risk assessments
  • Acting as our point of contact for the Leicestershire County Council brokerage team, responding to package offers promptly and managing capacity honestly
  • Maintaining accurate records on our electronic care management system, including care plans, medication records, visit monitoring and alerts
  • Monitoring missed and late calls, investigating and resolving issues, and escalating clinical concerns to the Registered Manager
  • Reviewing care packages, arranging and recording reviews, and updating plans when someone's needs change
  • Supporting carers in the field with spot checks, competency observations, supervisions and on-the-job guidance
  • Managing enquiries from service users, families, social workers, district nurses and hospital discharge teams
  • Contributing to audits, quality assurance and CQC evidence, and taking part in the on-call rota
What we are looking for
  • Previous experience as a Care Coordinator, Scheduler or Field Care Supervisor in domiciliary care
  • Level 3 Diploma in Health and Social Care, or equivalent, already held
  • Willingness to work towards a Level 5 Diploma and a Care Manager role, with our support
  • Direct experience of delivering care under a local authority framework, ideally the Leicestershire County Council Home Care framework
  • Confidence carrying out initial assessments and writing care plans and risk assessments
  • Established working relationships with council brokerage and social work teams
  • Competence with an electronic care management and rostering system, such as CareLineLive, Access, Nourish, Birdie or PASS
  • A driving licence and access to a vehicle
  • Strong written English, since much of this role is documentation that has to stand up to scrutiny
