Job summary

CCH Group in Cardiff is seeking a Care Coordinator to organise and deliver high-quality home care in the community. The role requires coordinating referrals, creating rotas, and liaising with social workers and GPs to monitor service users' conditions.

You will supervise care workers and conduct appraisals, building strong relationships across the care team. A driving licence is required, with Monday–Friday 9am–5pm hours and on-call duties on a rota basis.

• Experience & Organisation: You are an efficient, organized Care Coordinator passionate about community care.
• Stepping Up? if you have healthcare experience and understanding of CQC, care plans and rotas, we will support your transition.
